Referral process

I am happy to accept referrals from colleagues, either simply for advice or full periodontal and/or implant therapy.

If you would like to refer a patient please either send a normal referral letter, download a hardcopy referral form in WORD format or PDF format or complete the online referral form and send it to us. We will then endeavour to contact your patient as soon as possible to organise an appointment.

If you are a potential patient and would like to refer yourself for a consultation, please complete the referral form and send it to us or contact the practice directly.

Radiographs

It is useful for me if referring colleagues can send either original radiographs or copies along with their referrals, or can email digital images. This helps to streamline the consultation appointment for their patients and reduces radiation exposure. Further radiographs will be taken as needed, which I am happy to make available to referring colleagues for their records.

Periodontal report

As part of the referral service I will provide you with a report of my clinical findings and an outline of the proposed treatment. Wherever possible I try to involve the referring practice in the treatment of their patients and am more than happy to work in association with referring colleagues.

You can be assured that all patient details will be treated in the strictest confidence.
